Previously: A brilliant autistic orderly's keen observation saves a dying boy from a renowned surgeon's fatal error. But can he prove his diagnosis before it's too late?

The Orderly's Intervention

In that suspended moment, as Dubois prepared to plunge the needle into the boy's fragile chest, Samuel's hand shot forward. It was a blur of motion, his slender fingers clamping around the surgeon's wrist with surprising, desperate force. Tendons strained across his forearm, and beads of sweat popped along his hairline. "Do not plunge that needle!" Samuel gasped, his voice strained, urgent, and laced with a stammer he couldn't control. "Look at his fingernail lunulae—slate-brown, not blue!"

Dubois froze, his wrist locked in the orderly's vice-like grip. His jaw clenched tighter, his nostrils flaring with aristocratic fury. The surgeon remained silent, his eyes blazing at Samuel. He attempted to wrench his arm free, his chest heaving beneath the fine wool of his overcoat. The massive clockwork gears behind them seemed to grind in sympathy, their teeth meshing with a shuddering groan. "Insolent orderly!" Dubois spat, his voice dripping with venomous condescension. "Release my arm before I have you imprisoned for manslaughter!"

But Samuel didn't release his grip. Tears welled in his lower eyelids, not from fear, but from the overwhelming sensory input, the strain of holding firm against the surgeon's brute strength. His chin quivered, but his hand remained rigid. With painstaking effort, he peeled back Mateo's right eyelid, tilting the boy's head into the shaft of dusty sunlight. The golden rays struck the child's exposed eye, illuminating a distinct, iridescent golden-brown Kayser-Fleischer ring encircling the outer cornea. Dark chocolate-colored blood dripped sluggishly from an IV catheter inserted into the boy's arm.

"Golden Kayser-Fleischer ring in the cornea and chocolate blood!" Samuel declared, his voice now a rapid-fire, clinical torrent. "It's fulminant Wilson's disease with hemolytic crisis—epinephrine will rupture his myocardium instantly!" The surgeon's face drained of color, transforming into a ghastly chalk white. His eyes widened in paralyzed horror, the loaded syringe trembling violently in his now-loosening grasp.
